A reply to whoever sent it, naming the patient and what we are doing next. If we cannot help, you get that answer in the same reply rather than silence.

We ask for the letter of protection and confirm the attorney of record. Your office is not the middleman for that conversation.

To take the signed agreement and HIPAA authorization, explain what they are agreeing to, and arrange pickup or delivery. In English or Spanish, whichever they prefer.

Filled and delivered, or blocked and why — unreachable firm, firm declined to sign, patient has coverage they should use instead. Nothing sits in a queue unresolved.
Please do not put the prescription in the form
A prescription has to come from the prescriber by a lawful route — e-prescribed, or faxed from the prescribing office to (323) 653-4079. The form is for the referral: who the patient is, who represents them, and what they need. Sending the two separately is both faster and correct.
